WHY ARE SOME HOSPITAL FORMS UNAVAILABLE FOR VIEWING?
Unavailable hospital forms are in inventory but haven't been cleared for publication. As many hospital forms are sourced from medical litigation, patient data must first be erased before
forms can be cleared for web publication. For content-dense forms, digitally erasing patient data is an especially labor intensive process.
WHAT IS THE DIFFERENCE BETWEEN 'TYPESET', 'SCANNED' & 'BACKORDER' FORMS?
Typeset forms are the highest image-quality forms on this site. Scanned forms are the lowest image-quality forms on this site, and often originate from closed medical litigation cases.
Backorder forms are in inventory but haven't been cleared for publication.
DOES HOSPITAL-FORMS.COM SELL PAPER FORMS?
NO. We custom-build electronic electronic hospital-forms systems for institutional-grade clients. For smaller sized clients + individuals we offer forms typesetting services. If you find a form on this site you wish to have professionally printed, we recommend using Vista Print.
WHY DO 3 BANNERS DISPLAY AT TOP OF PAGE (INSTEAD OF 1)?
Typically, it's because of 1 of 2 reasons:
