 Probably.  Our editable Excel (.xls) forms work on most Mac & Windows operating systems with Excel software installed: may not work on Vista operating systems that use Excel's new 4 digit (.xlsx) extension.  BEFORE BUYING any editable Excel (.xls) form, verify that the ExcelTest test page opens & saves properly on your computer.
[2]Will editable Word (.doc) forms function on my computer?

[3]Why is there a $15 per page fee for Consult (.pdf) Forms?
 Consult (.pdf) forms originate from a different source than our Free (.pdf) forms, and take 5x more labor to format.  BEFORE BUYING any non-editable Consult (.pdf) form, verify ConsultTest image quality is acceptable.
[4]Why is there a 10 day backorder on all Consult (.pdf) Forms?
 Consult (.pdf) forms are inventoried when received in unprocessed format, and are reformatted at the time order is placed.  Standard turnaround for reformatting is 10 calendar days.
[5]How else do Consult (.pdf) Forms differ from Free (.pdf) Forms?
 In 2 ways. Where most Consult (.pdf) forms originated from JCAHO compliant hospitals in the 21st Century, exceptions were made for unique clinical content (plastic surgery protocols, for example) dating as far back as 1995 (+/- 10% of Consult (.pdf) inventory is pre-21st Century).

[A]Why are there duplicate FORM TITLEs and some forms labeled V1, V2, V3, V4 & V5?
 Duplicate FORM TITLEs occur when two or more hospitals have two or more forms with the exact same title. FORM TITLEs which contain V1, V2, V3, V4 & V5 embedded in their FORM TITLE denote multiple forms from multiple hospitals with the same title.
[B]What's the difference between "Typeset", "Scanned" & "Backorder" forms?
 Typeset forms can be easily edited, display superior image quality, and have a smaller file size for faster download. Scanned & Backorder forms cannot be edited, display inferior (grainy) image quality, and have a larger file size (slower download).
[C]Why can't I edit any of the FREE forms on this website?
 FREE Forms display in non-editable Adobe pdf format. Editable forms are now available for immediate sale in Excel (.xls) and Word (.doc) format. Backorder forms can be converted to editable format for prepaid orders only.
